Your Medical Home

A Medical Home is the health care facility (in most cases, a clinic) where you get health care. You choose your Medical Home when you enroll in and renew your Healthy San Francisco (HSF) coverage. You will get all your care through your HSF Medical Home.

Always call your Medical Home first when you have questions about your care.

Call your Medical Home to:

  • Make an appointment
  • Ask about refills for prescription medications
  • Get referred to a specialist
  • Enroll in or renew your HSF coverage
