Make an Appointment at Your Medical Home
If you are an HSF participant, please reach out to your Medical Home to make a doctor’s appointment.
You can locate your Medical Home address and phone number on your Participant ID Card.
To cancel or reschedule your appointment, you must call your Medical Home at least 48 hours before your scheduled appointment.
Please know that you may not be able to see your doctor or provider immediately, so try to plan ahead by making your appointment early when you know you need access to care.
