Where can I renew HSF?

To renew in the Healthy San Francisco Program you must make an enrollment appointment with a Certified Application Assistor (CAA). CAA’s are located in many locations throughout San Francisco.

To find an enrollment site, select a clinic from the pull-down menu below and call the phone number listed to make an appointment to complete a Healthy San Francisco application.
