Your ID Card

Healthy San Francisco (HSF) Participants are mailed an identification card called a Participant ID Card. The card will be mailed when you enroll in or when you change Medical Homes.

Your ID card tells your Medical Home that you are enrolled in HSF. Carry it with you at all times.
Your ID card is especially important when:

  • Visiting your Medical Home
  • Getting health care of any kind

Order a Replacement ID Card

If you lost your Participant ID Card, please call Customer Service to get a new one:
