Make the Most of Your Appointment
Your health matters to us. We want to make sure you make the most of your appointment. Try these tips as you get ready for your upcoming appointment(s).
Before visiting your Medical Home
Tell the person who is helping you make your appointment about your health concerns. This will help them make sure the right amount of time will be scheduled for your visit. If it’s your first visit to your Medical Home or with a new doctor, be ready to share your medical history. This means:
- Telling your doctor about any health conditions that may run in your family
- Telling your doctor all of your current and past health illnesses and treatments
- Bringing the containers (or take pictures) of any prescription and over-the-counter medicines, herbs, supplements, or vitamins you are taking
Write down a few things before you go to the doctor. It can make a BIG difference!
When describing a health problem, make sure to mention:
- A list of symptoms
- When the problem started
- Where it is
- What it feels like
- If there is anything you do that makes it worse or better
- A list of all medications/herbs/supplements/vitamins you are taking
(including how much and how often) - Any questions you would like to ask your doctor
While you are at your Medical Home
When talking with your doctor, remember that your health is worth their time.
- Clearly describe your health concern (Symptoms, when it started, where it is, what it feels like)
- Ask questions until you are sure that the doctor understands your concerns and has fully answered your questions
- Ask your doctor to write things down if you aren’t sure you will be able to remember everything
- Don’t forget about your emotional health – it affects your physical health. Make sure to mention all your mental health concerns, not just physical ones
Before leaving your Medical Home, find out:
- If and when you should come back for another visit
- If you need to call to learn about any test results or for a report on your condition
- If there are certain warning signs you should watch for
- If there are materials you could take home, or a website you should visit, to learn more about your diagnosis or treatment
Most people have trouble remembering everything their doctor says. It’s a good idea to write things down during and after your appointment so you can keep track.
Never leave your Medical Home without being sure about your diagnosis or treatment.
