STEP 2: Complete a Healthy San Francisco Application at your enrollment appointment.
At your enrollment appointment, you will meet with a Certified Application Assistor (CAA). Your CAA will help you apply for the program and verify your eligibility. If you don’t bring all required documents to your appointment, your CAA will be unable to complete your application, so be sure to bring them to your appointment. For the list of required documents, click here.
If you qualify, you will receive a Summary Sheet from the CAA with more information about your Medical Home and instructions on how to access medical services.
Within the next couple of weeks, you will receive a Participant ID Card and a Participant Handbook
in the mail.
If you are required to pay a Participant Fee, you will receive a bill in the mail within the next month.
