What To Know Before Your Appointment
- Your appointment may take 1 hour or more.
- Your appointment may be cancelled if you are more than 15 minutes late.
- If you are feeling sick, please call us at 1(415) 777-9992. We can reschedule or change your appointment to a phone call.
- For the safety of staff and other visitors, all visitors must wear a mask. If you do not have a mask, one will be given to you.
Required Documents
Each program has a different process of enrolling, but we’ll help you figure out each step. Watch this video guide on the documents you need.
- Have all your documents ready before your appointment. For the full list of documents you need, please click here.
- All documents (except for photo ID) must be dated within 45 days of your appointment date.
- Documents must show proof of income for EACH person in your household and proof of San Francisco residency.

- If you have an in-person appointment, bring your documents and photo ID. If you have a phone call appointment, have your documents ready to look at.

In-person appointments hours:
Tuesday and Thursday 8:30am – 5:00pm
Wednesday 8:30am – 4:00pm (closes at 3:00pm every 3rd Wednesday of the month)
Phone appointments hours:
Monday to Friday 8:30am – 5:00pm
Saturday 8:30am – 12:00pm
