Are you Eligible？
You may qualify for Healthy San Francisco if you are ALL of the following:
- San Francisco resident
- Age 18 or older
- Uninsured
- Not eligible for public insurance programs such as Medi-Cal or Medicare
- Living on a combined family income at or below 500% of the Federal Poverty Level.
Healthy San Francisco has created a streamlined portal called HSF Connect with resources to help you determine if you’re eligible for coverage. Within HSF Connect you can get pre-screened, fill out an eligibility questionnaire, apply by making an appointment, and upload important documents.
If eligible, you may join Healthy San Francisco regardless of immigration status, employment status or pre-existing medical conditions.
