Provider Reimbursement
Provider Grants
HSF Medical Homes have different models of payment.
San Francisco Health Plan (SFHP) is the third-party administrator for HSF. For HSF Medical Homes
and facilities (non-San Francisco Department of Public Health), SFHP manages:
- HSF provider agreements
- HSF grant payments
To receive payment, providers must submit their monthly invoices with the required information.
Invoices must have:
- Enrollment data
- Encounter data
Payment will be processed by the fifteenth (15th) of each month.
The POS fee amount is based on the:
- Participant’s Medical Home
- Participant’s household income
- Service being provided
