步驟 1：向持證申請助手 (CAA) 提出註冊預約。
若要申請 Healthy San Francisco，請在註冊地點進行預約。三藩市有許多便利的地點。

赴約時，您需要攜帶某些文件，以便：

步驟 2：在註冊預約時填寫 Healthy San Francisco 申請表。
在預約時，持證申請助手 (CAA) 將檢查您是否符合資格並協助您申請。您必須攜帶所有 必需文件才能完成申請。如果您未攜帶所有文件，CAA 將無法完成您的申請。
如果您符合資格，您將收到 CAA 傳送的摘要表，其中包含有關您的指定醫療診所的更多資訊以及如何獲得醫療服務的說明。如果您符合 HSF 資格，您的 CAA 將為您提供下一步指南。此下一步指南包含有關您的指定醫療診所的資訊，並會告知您如何獲得服務和護理。
您將在 7 到 10 個工作日內收到郵寄的 參與者 ID 卡和 參與者手冊 。 如果您需要支付參與者費用，您將在下個月內收到郵寄的帳單。
