Enrollment Information

STEP 1: Make an enrollment appointment with a Certified Application Assistor (CAA).

To apply for Healthy San Francisco, make an appointment at an enrollment site. There are many convenient locations in San Francisco.

To find one, either:

  • Choose a location from the menu below, call the listed phone number, and make an appointment OR
  • Download a printable version of appointment locations here.

You need to bring certain documents to your appointment to:

  • Make sure you are eligible
  • Complete your application

You can also get help to enroll in HSF at the SFHP Service Center. The Service Center provides application and enrollment support for many health programs in SF, like:

  • Medi-Cal
  • Covered California
  • CalFresh

STEP 2: Complete a Healthy San Francisco Application at your enrollment appointment.

At your appointment, a Certified Application Assistor (CAA) will check that you are eligible and help you apply. You MUST bring all the required documents to complete your application. If you don’t bring all your documents, the CAA will not be able to finish your application.

If you qualify, you will receive a Summary Sheet from the CAA with more information about your Medical Home and instructions on how to access medical services.If you qualify for HSF, your CAA will give you a Next Step Guide. This Next Step Guide has information about your Medical Home and will tell you how to get services and care.

You will get a Participant ID Card and a Participant Handbook in the mail within 7 to 10 business day. If you are required to pay a Participant Fee, you will get a bill in the mail within the next month.
