How do I Apply?

STEP 1:  Make an enrollment appointment with a Certified Application Assistor (CAA).

To apply for Healthy San Francisco,  make an appointment at an enrollment site. There are convenient enrollment sites throughout San Francisco. To find an enrollment site, select a location from the pull-down menu below and call the phone number listed to make an appointment.

In order to verify your eligibility and complete your application, you will need to bring certain required documents to your appointment. For a list of required documents click here.

The SFHP Service Center is also available to apply for health care options, such as Healthy San Francisco, Medi-Cal, and Covered California.

To make an appointment with an Enroller, please call 1(415) 777–9992  on Monday through Friday, 8:30am to 5:30pm. The Service Center may be available for last minute appointments based on availability. For more information about the SFHP Service Center, please access the SFHP website.

STEP 2:  Complete a Healthy San Francisco Application at your enrollment appointment.

At your enrollment appointment, you will meet with a Certified Application Assistor (CAA). Your CAA will help you apply for the program and verify your eligibility. If you don’t bring all required documents to your appointment, your CAA will be unable to complete your application, so be sure to bring them to your appointment. If you qualify, you will receive a Summary Sheet from the CAA with more information about your Medical Home and instructions on how to access medical services.

Within the next couple of weeks, you will receive a Participant ID Card and a Participant Handbook
in the mail.

If you are required to pay a Participant Fee, you will receive a bill in the mail within the next month.
